Airman and Family Program Enrollment
The following are checklists for parents interested in thesmse Airman and Family Services progra. Form names/numbers are linked.
Enrolling on CDC Waiting List
- Complete DD2606 (Request for Care)
- Email completed form to 1SOFSS.FSFC@hurlburt.af.mil
Enrolling in CDC
- Complete AF1181 (Program Patron Registration)
- Complete DD2652 (Application for Fees)
- Submit a copy of earning statements/pay stubs for all adult members living in the household
- Submit a current AF357 (Family Care Certification) Dual and Single Active Duty Military ONLY
- Submit a copy of current shot records
- Complete special needs paperwork (if applicable, i.e. documented food and insect allergies, speech/language disabilities, physical therapy, hearing loss, etc.)
Enrolling in SAP
- Complete AF1181 (Program Patron Registration)
- Complete DD2652 (Application for Fees)
- Submit a copy of earning statements/pay stubs for all adult members of household
- Submit a current AF357 (Family Care Certification) Dual and Single Active Duty Military ONLY
- Submit a copy of current shot records
- Complete special needs paperwork (if applicable, i.e. documented food and insect allergies, speech/language disabilities, physical therapy, hearing loss, etc.)
- Transportation Agreement (to/from school and field trips)
Enrolling in Open Recreation, Teen Program, and Instructional Classes
- Valid DoD Dependent ID Card
- Complete registration/parent contact form
